HospitalityCare Information Booklet – This booklet contains detailed and plan information for Owners/Management to review prior to deciding to enroll. This booklet is not intended to be distributed to employees as we customize open enrollment materials specific to each location once the Owner has decided to move forward with offering the HospitalityCare plan.

Participation Agreement - Prior to scheduling your enrollment, you must fax back a signed Participation Agreement. This Participation Agreement is non-binding and simply states your interest in offering our exclusive HospialityCare plan to your employees. You will then work with an account executive to gather information about your property and to discuss scheduling an enrollment day.
